To help us provide the highest level of service, please ensure this form is completed in full. It is essential to include the make, model, and serial number of the unit. Without this information, we are unable to accurately process your request.

If your unit is out of warranty, the service call will be billable. Unless you have a credit account with Stoddart, payment is required in advance via BPoint link or Direct Deposit for the call-out fee and the first hour of labour. Any additional charges (parts or extra labour) will be COD — payable onsite once quoted.

Important Information:
• Warranty service calls are available Monday to Friday, 7:30am–4:00pm (excluding public holidays)
• Requests outside these hours incur penalty rates and must be logged by phone at 1300 307 289, with written approval via email
• Certain components (e.g. glass parts and light fittings) are not covered under warranty
• Charges will apply for non-covered parts, "no fault found" diagnoses, travel beyond 50 km, or services outside our standard terms. These charges will be COD — payable onsite
